How to obtain your Medical Marijuana Card?

Schedule an appointment with our clinic. Prior to your visit, complete the electronic paperwork emailed to you by clinic staff.
Get Certified
Following your appointment, our clinic will submit the required Physician Certification Form to the State of South Dakota.
Apply for Your Card
Complete patient registration on the South Dakota Medical Cannabis Program website and receive your card within 30 days!

How To Get Your Mississippi Medical Marijuana Card
Step 1: Book An Appointment
- Call us at 601-336-2204 or schedule an appointment online.
Step 2: Provider Evaluation and Certification
- Complete the electronic paperwork emailed to you after booking your appointment.
- Meet with one of our providers to review your medical history, including qualifying conditions.
- Following your appointment, a certification form for the medical cannabis program will be submitted by the provider to the State of Mississippi through an online portal.
Step 3: Patient Application
- Within 60 days of certification by the provider, a patient submits an application to participate in the medical cannabis program.
Step 4: Receive Patient Identification Card
- The patient receives an electronic identification card for a time period specified by the certifying practitioner or a maximum of one year.
Step 5: Purchase Medical Cannabis
- Purchase medical cannabis from a licensed dispensary.
*Applications for a medical cannabis card cannot be processed until the Mississippi Medical Cannabis Program receives the certification for medical cannabis use by a practitioner.
Qualifying Medical Conditions
The following medical conditions or their treatment qualify for participation in the Mississippi Medical Cannabis Program:
- Cancer
- Parkinson’s disease
- Huntington’s disease
- Muscular dystrophy
- Glaucoma
- Spastic quadriplegia
- Positive status for human immunodeficiency virus (HIV)
- Acquired immune deficiency syndrome (AIDS)
- Hepatitis
- Amyotrophic lateral sclerosis (ALS)
- Crohn’s disease
- Ulcerative colitis
- Sickle-cell anemia
- Alzheimer’s disease
- Agitation of dementia
- Post-traumatic stress disorder (PTSD)
- Autism
- Pain refractory to appropriate opioid management
- Diabetic/peripheral neuropathy
- Spinal cord disease or severe injury
Also qualifying is a chronic terminal or debilitating disease or medical condition or its treatment that produces one or more of the following:
- Cachexia or wasting syndrome
- Chronic pain
- Severe or intractable nausea
- Seizures
- Severe and persistent muscle spasms including, but not limited to, those characteristic of multiple sclerosis
Medical marijuana patients must meet the following qualifications:
- Have a current diagnosis of a qualifying condition.
- Be a Mississippi resident with proof of residency (valid South Dakota driver’s license or state ID card)
- For patients under 18 years of age, evaluation/certification must be done by a physician only.
- For patients 18-25 years of age, evaluation/certification must be completed by two providers from separate practices, one of which must be a physician.
Qualifications and Limitations for Caregivers:
- Must be 21 years of age or older, unless they are a parent/legal guardian of the qualifying patient.
- Must have their own Patient Identification card.
- Must undergo a background check.
- Prohibited from having a disqualifying felony offense.
- Expires after 1 year (unless assisting a non-resident patient).
